Our program helps people use and find their way through the aged care system and other community-based services.
Care Finder facilitates the connection between individuals and the necessary support systems while ensuring continued connectivity.
This program can help with:
- Guiding individuals through the care assessment process, accompanying them through any necessary assessments to fulfill their requirements
- Direction and clarification regarding the care assessment process
- Finding and short-listing aged care providers in their area
- Completing forms and understanding aged care service agreements
- Checking-in once services are up and running to make sure everything is okay
- Connecting to services in the community, such as health, mental health, housing and homelessness, drug and alcohol services and community groups.
To receive Care Finder Support through genU, a person must:
- Not have a carer or support person who can help you.
- Are over 65 or over 50 for First Nations people.
- Over 50 or 45 for First Nations people on a low income and homeless or at risk of homelessness.
- Live in City of Greater Geelong, Surf Coast Shire or Borough of Queenscliff
- Having difficulty accessing service.
